Telephone NDIS Support Coordinator
Location: Work from Home
Hours: Casual, Full-Time, Part Time

Role Overview
As a Support Coordinator, you will:
Assist participants in coordinating and implementing supports and therapies in line with their NDIS funding and goals.
Help participants engage with mainstream supports and maximize their NDIS plans.
Support plan reviews and submissions for changes in circumstances.
Facilitate home modification applications and assistive technology requests.
Build capacity and understanding of the NDIS processes.
Remote Work: Enjoy the flexibility of working from home with the support of a strong team culture.
Supportive Environment: Be part of a small, dedicated team that prioritizes individual needs and approaches.
Requirements
To be considered for this role, you must have:
Your own ABN
100 Points of ID
Valid visa
Your Own Personal phone, laptop, and Wi-Fi access
Basic computer skills
Police check completed within the last 6 months
NDIS Worker Screening including Blue Card (to be obtained before commencing)
Completion of the NDIS Worker Orientation module
